To begin with, select Forms in your sidebar and click the “+Forms” button.
Add a name for your Form and select a list you want the data to propagate into once the user fills in and submits it.
Now you will be redirected to the Form builder where you see options like Welcome Screen, Questions, Thank you Screen, and Settings.
The Welcome Screen is the first screen the user will see after opening your form. Here, you can add an Image via a URL or upload it from your computer which will be embedded in the welcome screen. Ensure that the file size is under 10MB.
You can also add a description that will appear below the image and a CTA button.
Next, you have to add questions that you want your users to answer. You can add as many questions as you like and reorder them with a simple drag & drop.
After that is the Thank you screen where you thank your users for filling out your form
Lastly, the settings allow you to customize your form as per your preference. You first have to select a list for storing the data entered into the form, followed by the Owner.
The submission will be assigned to the selected team member automatically. If you select more than one Team Member, it’ll be assigned on a sequence basis.
After that, you can customize the background image of your form and change the color settings to match your brand’s personality.
Once done, click Publish to receive the code for the Form, which you can share however you want.
